In accordance with the California Public Records Act (Gov't Code  §6250-6268), I'm requesting copies of the following records related to the tenure of Antonio Villaraigosa as Mayor of Los Angeles from July 1, 2005 through June 30, 2013:

Reimbursements: All reimbursement records of submitted by former Mayor Villaraigosa. Responsive records should detail the amount of the requested reimbursement, amount reimbursed, reason for the request, supporting documentation, and the date of the reimbursement.

Office-Related Expenses: I am request copies of all office-related expenditures of former Mayor Villaraigosa’s office. Responsive records should detail the date an expense was incurred, the cost of an expense, and a description of the expense (i.e. furnishings, technology, office supplies, food, etc).

Calendars, Meeting Planners, and Scheduling Materials: I am requesting copies of all calendars, meeting planners, and scheduling materials of former Mayor Villaraigosa. This includes electronic or paper calendars and meeting invitations.

Travel/Advance Logs and Itineraries: I am requesting copies of all travel/advance logs itineraries of former Mayor Villaraigosa. Responsive travel records should detail the date and time, the destination, the intended purpose, names of those who accompanied, and whom former Mayor Villaraigosa met.

Vehicle Expenses and Logs: I am requesting copies all expenses, reimbursements, and logs related to former Mayor Villaraigosa’s usage (or member of family’s usage) of any publicly-owned/leased. Travel logs should detail the date and time of usage, the destination, the number and identity of the occupants, the model of the vehicle, and the estimated distanced traveled as available.

Plane/Helicopter Expenses and Logs: I am requesting copies of all expenses, reimbursements, and logs related to former Mayor Villaraigosa’s usage (or member of family’s usage) of any publicly-owned/leased plane or helicopter. Travel logs should detail the date and time of usage, the destination, the number and identity of the occupants, the model type, and the estimated distanced, and the value of trip traveled as available.

Public Law Enforcement Usage Records: I am requesting copies of records that detail all the use of law enforcement units assigned to accompany/protect former Mayor Villaraigosa. These records should detail the number of officers assigned, the date when they were assigned, the hours of security protection provided, and the estimated cost of such services being provided.

Private Security: I am requesting copies of all contracts, invoices related to any private security services provided former Mayor Villaraigosa. Also include all related reimbursement requests submitted by former Mayor Villaraigosa (or staff) for such security services being provided.

Salary & Other Compensation: I am requesting copies of records that detail former Mayor Villaraigosa salary, deferred salary, retirement benefits, and other compensation (including insurance plans).

Personnel Record: I am requesting a copy of former Mayor Villaraigosa’s personnel file. These records should include the duration of employment, the position held, disciplinary history, and the annual salary.

Gifts: I am requesting copies of all gifts made to former Mayor Villaraigosa directly and former Mayor Villaraigosa office. These records should include the name of the giver, the estimated value of the gift, the date given, and the description of the gift.

Email Correspondence: I am also requesting copies of all releasable incoming-and-outgoing electronic correspondence of former Mayor Villaraigosa’s assigned email accounts.

I authorize you to incur up to $100 in fees to fulfill this request. If the cost of fulfilling this request will exceed that amount, I request that you provide me with a written cost estimate for making these records available to me prior to incurring any costs. 

My preferred reproduction format is an electronic file e-mailed to me at mperch87@gmail.com. If this is not possible, I request that you provide access to these records electronically via an FTP site, or mail electronic copies of the records to me on removable media, such as a CD or flash drive. If none of these delivery methods are feasible, I request that you mail paper copies of the records to me at the address provided above.
