Pursuant to my rights under the California Public Records Act (Government Code Section 6250 et seq.), I ask to obtain a copy of the following, which I understand to be held by your agency:

 

(1) All City of Los Angeles records, correspondence, and/or communications, (including written documents, e-mails, photos, diagrams, videos, audios, voicemails, computerized data) that refer or relate to the injury of any persons which occurred on or about August 1, 2016 at or near 4902 Collis Avenue, South Pasadena, CA 90042.

 

(2) All City of Los Angeles records, correspondence, and/or communications, (including written documents, e-mails, photos, diagrams, videos, audios, voicemails, computerized data) that refer or relate to the injury of any persons which occurred on or about August 1, 2016 at or near Los Angeles Metropolitan Transit Authority Bus Stop ID No. 10311, Los Angeles, CA 90032.

 

(3) All City of Los Angeles records, correspondence, and/or communications, (including written documents, e-mails, photos, diagrams, videos, audios, voicemails, computerized data) that refer or relate to the injury of any persons which occurred on or about August 1, 2016 at or near Los Angeles Metropolitan Transit Authority Bus Stop ID No. 11082, Los Angeles, CA 90032.

 

(4) All City of Los Angeles records, correspondence, and/or communications, (including written documents, e-mails, photos, diagrams, videos, audios, voicemails, computerized data) that refer or relate to written complaint(s) made by Rodolfo Sotomayor within the 10 calendar years prior to August 1, 2016 (August 1, 2006 forward).

 

(5) All City of Los Angeles records, correspondence, and/or communications, (including written documents, e-mails, photos, diagrams, videos, audios, voicemails, computerized data) that refer or relate to tree trimming, arborists, debris clean-up, sidewalk repair, utility repair and/or general maintenance to the area in front of 4902 Collis Avenue, South Pasadena, CA 90042 within the previous 10 years.

 

(6) All City of Los Angeles records, correspondence, and/or communications, (including written documents, e-mails, photos, diagrams, videos, audios, voicemails, computerized data) that refer or relate to tree trimming, arborists, debris clean-up, sidewalk repair, utility repair and/or general maintenance to the area at or near Los Angeles Metropolitan Transit Authority Bus Stop ID No. 10311, Los Angeles, CA 90032 within the previous 10 years.

  I ask for a determination on this request within 10 days of your receipt of it, and an even prompter reply if you can make that determination without having to review the record[s] in question.  I attach proof of my identity.  Please provide an estimate of the cost.
